full refund, except for the membership activation fee where permitted by law. This contract is not protected by any life and health guarantee
association. The program and program administrators are not liable for providing or guaranteeing health services or for the quality of health
services rendered. Membership and activation fees apply. This program is governed by the Membership Agreement provided upon activation.
